To all incoming freshmen students who wish to enroll in the College of Health Sciences Education (Nursing, Medical Technology, Pharmacy, and Nutrition Dietetics), please be informed of the following requirements for your medical certificate. You are required to bring the results of the following laboratory tests:
Please submit the results to the UM Health Services/Clinic, Embassy Area, Bolton St., Davao City, to obtain your medical certificate as part of your admission requirements.
